Lifehouse Youth 2025 Rego

Lifehouse Youth is a place for teenagers in grade 7-12 to connect, study the bible and grow in their faith together. In 2025, this will be a connect group style, held at either Lauren and Eddie's house or church, on a weeknight 6:30-8pm.

We, Eddie and Lauren, lead the youth, sometimes with the help of other Lifehouse Church members who have all undertaken childsafe training and WWVP checks. Please note, that in the event that a youth attendee turns 18 (year 12 aged) they do not automatically become leaders, this is only under appointment.

This permission form exists to evidence your knowledge and approval of your child to attend this weekly connect group, and for leaders to obtain all necessary information to safely care for your child.

Medical Authorisation

Should I/we be unavailable in the case of an emergency involving my/our child. I/We hereby authorise the Leaders to arrange for such Medical or Surgical treatment, and the administration of medication such as analgesics, as may be deemed necessary. Due to occassionally being offsite, we will need to transport children to and from the venue, in these events we may hire a bus or occassionally transport a small portion by car. Leaders will always abide by the law and have licenses ranging from probationary to full licenses.
